Transition Tuesdays Community of Practice

Transition Tuesdays is a Community of Practice series for Special Education Teachers and Itinerant Staff who support transition-aged students and Young Adults. Participants will help shape future topics through surveys, ensuring the series remains relevant, collaborative, and focused on improving postsecondary outcomes for students with disabilities. Sessions provide training updates, upcoming event announcements, compliance reminders related to Indicator B13, guest speakers from community agencies and colleges, and practical strategies for developing work-based learning opportunities.

1. Identify current requirements and county-wide common errors related to Indicator B13 transition compliance.
2. Apply effective transition planning practices that support positive postsecondary outcomes for students with disabilities.
3. Utilize resources and information from community agencies, colleges, and other transition partners to strengthen transition services.
4. Develop and expand work-based learning opportunities within their district or program.
5. Implement strategies shared through the Community of Practice to improve transition services and student outcomes.
6. Access and share current transition-related resources, training updates, and professional learning opportunities.
7. Collaborate with colleagues to address challenges and share promising practices in transition programming.
