Completion of the 2025-2026 Elwyn ELS Annual Path

You may request Act48 credits for completion of the Annual Path Trainings. In order to be eligible to request credit you must be a certificate holder with PDE. By completing this request you are confirming that you have completed all sessions in the Annual Path by June 30,2026. The total amount of hours for the 2025-2026 ELS Annual Path equates to 9 Act 48 credit hours. This request form must be completed within the dates of July 27, 2026 to COB on Aug 10, 2026.
Important: Partial credit is NOT permitted. Individuals who have not completed each of the courses in the annual path will not be considered for Act48 credits.
Requests that are not received within the timeline stated above will also not be granted.
